- Upon a face-to-face request, the client/guardian may receive a print out of the Face Sheet (displaying demographic and diagnosis information), Medication List, and/or Current Treatment Plan at no charge. Form completion is not required for this general request.
- If you send in a request, please allow up to 30 days (as permitted by HIPAA) for processing time.
- DO NOT leave multiple voicemails or emails as this will delay processing time.
