To request a copy of your medical records for yourself or to request ReDiscover to send copies to another healthcare provider, an insurance company, attorney, school, or other recipient, complete the Client Request for Health Information form.

You can come into any of our office locations and fill out the form with the front desk, OR, you can complete the form on your own and send it to us by one of the methods below:

For adult clients (18+ years of age): Under most circumstances, you can complete the form yourself. If you have a legal guardian or personal representative, they may also authorize the request on your behalf. A legal guardian or personal representative must provide proof of guardianship/appointment when turning in the completed form.

For minor clients (under 18 years of age): Unless the minor is emancipated, the parent or legal guardian must complete the form. If legal guardian is not the parent, proof of guardianship/appointment must be provided when turning in the completed form.

Records to Self (paper, electronically delivered, or pick-up @ ReDiscover location): First 10 pages free, $0.40 per page thereafter.

Records to Others (per MO state fees, effective 2/1/22):
Paper: $27.46 plus $0.63 per page and $25.71 for additional costs if records are maintained off-site.
Electronically delivered: $27.46 plus $0.63 per page, or $120.32 total, whichever is less.

Healthcare Provider: Free

The deceased client’s appointed Executor of Estate or Court Order Administrator must complete, sign, and date the request form. If there is no Executor of Estate or Court Order Administrator, a notarized Affidavit of Next of Kin may complete the form. Proof of appointment must accompany the form. Additional proof may be requested, as appropriate (i.e. death certificate).
