ReDiscover's Psychiatric Rehabilitation Services
CHANGING LIVES

Our comprehensive Psychiatric Rehabilitation Services help adults age 18 and over struggling with the challenges of mental illness find ways to achieve personal well-being. The program works with people to enhance the quality of daily life and take the steps needed to meet personal goals.

ReDiscover provides case management, medication management, counseling and support for adults with serious mental illness such as major depression, bipolar disorder, or schizophrenia.

Individuals with severe and persistent mental illness may also struggle with chronic medical conditions such as obesity, diabetes, and cardiovascular disease. ReDiscover helps to identify separate medical issues and link clients to appropriate care and treatment. Clients are encouraged to improve their physical health by taking part in exercise groups or working with a wellness coach in ReDiscover's Healthcare Home.

If someone is lacking basic necessities such as clothing, housing, transportation, or food ReDiscover will connect them with appropriate community resources. 

ReDiscover's employment specialists are available to provide employment resources to clients interested in finding a job in the community. They help clients with resume writing, skill building and interview techniques.

  • Monday - Friday, 8:30 AM - 5:30 PM
  • weekends as needed
